If you or they use Claude or ChatGPT
If you help manage their profile, you can connect your own Claude, ChatGPT or Grok to it and choose what it can see. Family members who can view the profile can connect theirs with read-only access. If they already use one and would like to, they can still connect their own, and ask about their records in their own words.
Step-by-step guides for ChatGPT, Claude, Grok, Muse and others
Within what each connection allows, the assistant can read their records, medications and the summary of their history. It works only with what is in Triangle.
- “Which medicines is Mom taking now, and what is each one for?”
- “Put Mom’s medical history into a few sentences I can share with the family.”
https://app.trianglehealth.com/mcp- Copy the address.
- In the assistant’s settings, add a new connector with it.
- Sign in to Triangle when asked.
- Choose what it may see and which profile to share, then Accept.
In Claude
Settings→Connectors→Add custom connector→paste the address
A Triangle sign-in appears the first time Claude uses a Triangle tool.
In ChatGPT
Plugins→+→Add custom MCP server
Enter a name and the address, choose OAuth, then Create as a plugin and sign in to Triangle. This needs ChatGPT Business, Enterprise or Edu, or Pro for read-only.
In Grok
Skills and Connectors→New Connector→Custom
Enter a name and the address, then sign in to Triangle and accept. This needs a paid Grok plan.
What an outside assistant reads is handled under that assistant’s own privacy policy, which Triangle does not control, so connect only one you trust. Each person sees and disconnects their own connections in Triangle under Settings → AI service connections, and access stops straight away.
Everything above starts with a profile for them.
