Evenfall Free trial

For families caring for someone in hospice

You're the one everyone asks. Keep their records in one quiet place.

Evenfall gathers your loved one's medical records into one history, explains them in plain words, and lets you share it with family and their doctors. When someone asks, the answer is in one place.

What it is like

The questions come from every side

A visiting nurse asks what they're taking. A brother asks what the last scan said. A form wants the year of a diagnosis made long ago.

The answers are spread across clinics, portals and a folder on the kitchen table.

How it works

Four steps, taken at your own pace

  1. Set up a profile for them

Begin your free trial and choose to set up a profile for someone you look after. Say where they live, and choose to connect their records.

  2. Confirm, sign and book

Confirm identity and address, sign the consent forms, and book an intake visit with a clinician.

  3. Let their records come together

Triangle requests their records through the national health-record networks. Add what is already in the folder: discharge papers, medication lists, photos of paper records. It all becomes one history, which updates as new records arrive.

  4. Share it with the family

Invite family members and their doctors. Choose who can see the profile, and who can help manage it.

Their own assistant

If they use Claude or ChatGPT

Only the person whose profile it is can connect an assistant such as Claude, ChatGPT or Grok. You can’t connect one for them, even if you help manage their profile. If they already use one and would like to, they can connect it to their own profile, and ask about their own records in their own words.

Within what they allow, the assistant can read their records, medications and the summary of their history. It works only with what is in Triangle.

  • “Which medicines am I taking now, and what is each one for?”
  • “Put my medical history into a few sentences I can share with my family.”
The connector address

  1. Copy the address.
  2. In the assistant’s settings, add a new connector with it.
  3. Sign in to Triangle when asked.
  4. Choose what it may see and which profile to share, then Accept.

Then, back in Apps & Connectors, choose Create under Connectors and enter a name and the address. This needs ChatGPT Plus, Pro, Business, Enterprise or Edu; the free plan can’t add custom connectors.

Enter a name and the address, then sign in to Triangle and accept. This needs a paid Grok plan.

What an outside assistant reads is handled under that assistant’s own privacy policy, which Triangle does not control, so connect only one you trust. It can be disconnected at any time in Triangle under Settings → AI service connections, and access stops straight away.

Everything above starts with a profile for them.

Trust

Private, and kept that way

Their records are encrypted, never sold and never shared for advertising. You decide who sees them, and they are yours to delete.

No drug maker, laboratory or hospital pays Triangle, the company that makes Evenfall. Members do. Triangle is HIPAA compliant (see its Trust Center) and is listed in the Medicare App Library.

Triangle provides research and educational support. It does not diagnose, treat, or prescribe, and it never replaces your care team.

Questions

What families ask

Is Evenfall a care service?
No. Evenfall doesn't provide care, diagnose or prescribe, and no one at Evenfall watches over them. Their hospice team is still the one to call. Evenfall keeps their records together and helps the family understand them.
How do their records get in?
Triangle requests them through the national networks, Carequality and TEFCA, which reach many hospitals and clinics, though not every one. Setting up asks for an identity check, the consent forms and an intake visit with a clinician. Anything the networks can't reach, you can upload: discharge papers, medication lists, photos of paper records.
What does it cost?
It begins with a 7-day free trial, which needs a card. If you cancel before day 7, you're not charged. After that it is $9 a month, and you can cancel any month. Membership covers requesting their records from hospitals and clinics and keeping them up to date as new records arrive, a chat that answers from their records, a review of their medical history by a licensed clinician, connecting wearables, and uploading anything else you have.
Who can see their information?
You decide who can see the profile and who can help manage it. Their records are never sold or shared for advertising.
Can it help with an advance directive?
It doesn't prepare documents or give legal advice. If you would like a copy kept with their records, you can upload it like any other document.
What happens to the records later?
They stay private. You can delete them whenever you are ready.
